This form is used to accompany New York State's Sexual Harassment Prevention Training – 2023. While watching the training video, please note your answers to the case studies and exercises in this form. 

Once you have documented your answers for all case studies and exercises, you will be prompted to review and sign IC's sexual harassment prevention policies and campus resources documents. 

  • You may save your progress and return at any time by select "Save and Resume Later" at the bottom of the form.

  • The Bias Impact Reporting Form is intended to provide students, staff, and faculty with a centralized way of accessing resources if they experience or witness a bias incident on campus, via social media, or at a college-affiliated event. This process is a tool that is being used in addition to formal complaint options.
